Batch Shirt, etc.

2012 Batch Shirt

This is the final design for our batch shirt!

Order forms will be passed out soon. Please accomplish them all by class ASAP. Please keep in mind that the batch shirt is required. :D 
It costs P250/shirt, same as last year's. 

Give all shirt payments to your respective class treasurers. Class treasurers, give the money to Kim Ednalino, IV-Electron.
Please settle all accounts by September 5.

OJ Acopiado: Paki kalat na yung mga nag-order sa form outside caf, payment is due on Monday. Paki-balot ulit sa papel or ilagay sa envelope, tapos ilagay yung inorder nila and amount. Thankss.

STR Lab Rules

We have 2 new rules for work that starts this week in the lab:

1. No lab gown, no entry, no lab work. STR will not lend lab gowns starting this week. Please bring your own lab gown. This will encourage you to always wear routinely, washed lab gowns.

2. Glassware, equipment, chemicals, etc. must be requested a day before you do the lab work. Please sign out for materials tomorrow for lab work to be done STR period on Thursday.

Instructions on how to apply in De La Salle University:
  1. Fill out the Online Undergraduate Application Form and print a copy.http://www.dlsu.edu.ph/admissions/undergraduate/applicationformnew/Introduction.aspx
     2.   Submit the completed printed Undergraduate Application Form with all the required documents at the Guidance Center:
 
          a.  Certified Correct Secondary Scholastic Records (please submit this on/or before August 15, 2011 to give ample time to the Registrar's Office to accomplished the form) download from http://www.dlsu.edu.ph/admissions/undergraduate/_pdf/secondary-scholastic.pdf
      b. Two (2) recommendations from any two of the following (deadline is August 23, 2011):) happy
Use the DLSU Recommendation Form (download fromhttp://www.dlsu.edu.ph/admissions/undergraduate/_pdf/recommendation.pdf)
  • High School Principal
  • Class Adviser
  • Guidance Counselor
          c. Three (3) identical passport photographs (available already at the Guidance Center)
          d. Photocopy of National Statistics Office (NSO) Birth Certificate for Filipino citizens or photocopy of Birth Certificate, ACR/IR or passport       
              for non-Filipino citizens
 
   3. Application fee is P600.
 

About the ACET Forms:
A parent is willing to go back to ateneo on Friday to submit the ACET Forms for those who have NOT submitted yet. Please give your completely filled out forms to the Guidance Center by Thursday,August 11, 2011, 5:00 p.m.
